Community partnerships normally run from July 2012– June 2013. However, if you join now, your partnership will begin now and last through June 2013. Additionally, the first 50 people to return this form will receive a 2011 Fiesta t-shirt (sizes are first come, while supplies last).

Silver $25-49
Gold $50-99
Platinum $100+
